Accident and Injury Reporting

In the event of an injury to any player or a registered AYSO Volunteer, please call the Safety Director immediately to file a report. The Safety Director will assist you in completing the proper forms for filing with AYSO’s insurer. The forms must be submitted to AYSO within 30 days of the accident / injury, so please call quickly.

Coaches don't forget: Players who aren't registered aren't covered by AYSO's insurance. Make sure you have a registration form for every player and keep those forms with you at all practices and games. If you have a player without a registration form, call the Regional Commissioner or Registrar immediately (429-2976).
